You Must File A Termination Ethics official looking over an OGE-278 formOGE-278 Report

You are going to be required to submit a termination OGE-278 form (Public Financial Disclosure Form) to OGC/Ethics within 30 days after leaving the EPA. Please note that your termination report cannot be filed earlier than your last day at EPA.

If you don't file on time, you can be assessed a $200 late filing fee. If you don't think you will be able to make the deadline, you must ask OGC/Ethics for an extension BEFORE your deadline expires. The maximum extension is 90 days. Failure to file a termination OGE-278 is punishable by a file of up to $50,000 and/or one year in jail.

A true EPA story: Once, a person left EPA but did not file a termination report. For nearly 2 years, OGC/Ethics tried to get this person to file a report and pay the $200 filing fee. Finally, EPA contacted the Department of Justice (as required by regulation) and DOJ filed a civil complaint. As a result, the ex-EPA employee paid a civil fine of $15,000

When filling out your termination OGE-278 form, remember to include your termination date and use your current EPA phone number and address. However, do give OGC/Ethics your contact information (phone number, email and address) should we need to contact you in the future.
